Youth Workshop Registration Information

FOR AGES 8-12 June 21 - 25, 2010 and July 12-16, 2010 AT THE ESTHER SIMPLOT PERFORMING ARTS ACADEMY
9:00 am - 3:00 pm daily

Workshop registration fee is $175.00.
Please Make Checks Payable to: mdt workshop
$50 cancellation fee.
For the June Workshop, no refunds after June 11, 2010.
For the July Workshop, no refunds after July 2, 2010.

Workshop fee must be included with your registration form to reserve your place and returned to:
